Neal Kwong was a Falcon Heights resident who died tragically from carbon monoxide poisoning due to a faulty furnace at the age of fifteen. He was a volunteer in our Parks and Recreation Junior Leaders Program, an Eagle Scout, and a great young citizen. In his memory, the City established this award to recognize people between the ages of 12 and 19 who display outstanding leadership, volunteerism, or citizenship in this community.

The award recipients will be recognized at a Falcon Heights City Council meeting and in the City newsletter. In addition, the award recipient's name will be engraved on a permanent plaque in the lobby at City Hall. 

Requirements

  • Must be a youth between the age of 12-19
  • Must reside in or complete volunteering work within Falcon Heights

Please feel free to make additional copies and nominate more than one young person who deserves recognition. Parents can nominate their own children as well as other children.

Application

There are two ways to nominate an award recipient:

  1. Download the application, complete, and return to City Hall via mail or email to mail@falconheights.org.
  2. Complete the application form online

Nominations for the 2025 Award are due no later than Friday, April 25th at 4:30 pm.
